After activating the plugin, you will see a note at the top of the Administration Screen asking you to add two constants, copy the values and paste the copied content into wp-config.php. Reactivate the plugin after editing wp-config.php. You must add _fm\_field-_ as a prefix to the beginning of the form-tag name in the _Form_ tab panel.
